Duties of the Community Mental Health Nurse: Responsible for the delivery of assessment, brief treatment and/or referral onwards to mainstream services Act as link workers providing support and advice to workers in those projects and operate clinics as necessary You will work closely with the NMP and Peer Support Worker in the team to offer a new and robust service to this excluded group, many of whom will have dual diagnosis and personality disorders Assess, plan, monitor and evaluate care of a designated caseload of homeless service users who have complex care needs and who are living in the Lambeth Vulnerable Adult hostels Attend reviews, case conferences, network meetings, ward and community based multi-disciplinary hearings, Mental Health Review Tribunals and Managers Hearings as a representative of the department Requirements of the Community Mental Health Nurse: Degree or equivalent in Mental Health Nursing Experience working as a Community Psychiatric Nurse NMC Registration Experience working in an NHS setting Experience working in a mental health or homelessness setting Contact: This Community Mental Health Nurse job is advertised by Deane Garrard; if you are interested in this position please click above to apply now.
