Job responsibilities: - Provide comprehensive nursing care to individuals and families in the community - Conduct health assessments and develop personalized care plans - Administer medications and treatments as prescribed - Monitor and evaluate patient progress and adjust care plans accordingly - Educate patients and their families on health promotion, disease prevention, and self-care techniques - Collaborate with other healthcare professionals to coordinate patient care - Document patient information accurately and maintain confidentiality Skills: - Strong clinical assessment and critical thinking skills - Excellent communication and interpersonal skills - Ability to work independently and as part of a multidisciplinary team - Proficient in using electronic medical records systems - Knowledge of community resources and support services - Compassionate and empathetic approach to patient care - Strong organizational skills and attention to detail
