Our client is a non-profit organisation working with people to support their health and wellbeing. Established for over 30 years, they have provided services alongside a variety of statutory and voluntary, community and social enterprise (VCSE) colleagues in Surrey and its borders. They work in Partnership with Surrey Borders Partnership NHS Foundation Trust and are currently looking for an enthusiastic, caring, organised Reconnect Assertive Outreach Worker to work within the Surrey Liaison and Diversion Service. The role provides Outreach Support in the Surrey Community as well as attending the five Surrey prisons to assess and support prior to release. Salary: up to GBP27,000 Per Annum, dependent on experience Hours: 35 hours Duration: Permanent Company Benefits: 26 days annual leave (rising with service) + bank holidays + 1 extra days leave on your birthday, Pension, Access to Simply Health, Blended working, Employee discount programme and more hellip;.. Location ndash; The service covers clients within the Surrey area, with the main office currently based in Guildford. The role involves blended working, travel to visit clients in the Surrey area ndash; (example Woking, Guildford, Staines, Chobham, Runnymede, Redhill etc). Travel expenses including mileage are reimbursed. Would suit car owner driver. The Assertive Outreach Worker will work alongside colleagues in the Liaison and Diversion Service to offer person-centred and community based support and interventions, aiming to improve the health and social care outcomes for individuals with the ultimate goal of reducing offending. Although primarily based within the community there will be some in prison working. The aim of working within the prison is to assess and build relationships with people who need support prior to their release. Reconnect will work with an individual for up to six months in the community providing intense support and continuity of care. The Liaison and Diversion Service is passionate about improving the lives of individuals by providing supportive early intervention for vulnerable people from the age of 10 who come to the attention of the criminal justice system and those who are aged over 18 years and require support on release from prison. The Liaison and Diversion Service provides assessments to identify vulnerabilities such as: mental health concerns, physical health concerns, learning disabilities, substance misuse concerns, housing, finance concerns, and other vulnerabilities. The Reconnect Assertive Outreach Team are able to offer and provide support for the identified vulnerabilities which can involve signposting, referrals to appropriate support services, assistance with making and attending appointments based on their needs, collaborative working with other services and organisations. The Reconnect Assertive Outreach Team also provides well being support in a person centred holistic way. This post is subject to an enhanced DBS check and full prison vetting. Our ideal applicant will have/be; Enthusiastic, with a drive to provide excellent person centred care, support and planning Willing to learn, and seek to improve quality of care Previous community experience working with individuals relating to mental health, learning difficulties, drug and alcohol addition, homelessness etc Knowledge of the criminal justice system and / or Health and Social Care is desirable Previous experience working within a prison is desirable Ability to work collaboratively within a multidisciplinary team Working knowledge of assertive outreach work, assessing and managing risk and safeguarding, policies and procedures supporting these Good communication skills both written and verbal Proficient in Microsoft Word, Outlook and Excel, with previous experience of an electronic patient record system preferrable Ability and willingness to travel and work flexibly eSift have been appointed as the resource partner and we are managing this role on behalf of our client.
