Role Overview: In this role, you will play a crucial part in ensuring the smooth operation of our care services, coordinating schedules, and providing support to both clients and care staff. Key Responsibilities: middot; Coordinate care services for clients, including scheduling appointments, arranging transportation, and coordinating caregiver assignments. middot; Conduct assessments to determine clients #39; care needs and develop individualized care plans in collaboration with healthcare professionals and family members. middot; Communicate with clients and their families to ensure their needs and preferences are met and address any concerns or issues that may arise. middot; Monitor the delivery of care services, ensuring adherence to care plans and regulatory standards. middot; Provide support and guidance to care staff, including training, supervision, and performance evaluation. middot; Maintain accurate records of client care plans, schedules, and communication logs. middot; Collaborate with healthcare professionals, social workers, and other stakeholders to ensure continuity of care and effective coordination of services. middot; Participate in care coordination meetings and contribute to the development of policies and procedures to improve service delivery. Qualifications Experience: middot; Level 4 NVQ in Health Social Care. middot; Previous experience in care coordination, case management, or a related role in the healthcare or social services sector with not less than 18 months in the United Kingdom. middot; Strong interpersonal and communication skills, with the ability to effectively interact with clients, families, and care staff. middot; Knowledge of community resources and support services available to individuals in need of care. middot; Excellent organizational and time management skills, with the ability to prioritize tasks and meet deadlines. middot; Compassionate and empathetic attitude towards clients and their families. middot; Ability to work independently and as part of a multidisciplinary team. middot; Attention to detail. middot; Proficiency in Microsoft Office applications and electronic health record systems. middot; Flexibility, organization, and a positive approach to challenges. middot; Owned car with a clean driving record.
