Job Description: As a Streamer/Assessor Nurse, you will be responsible for initial patient assessments, streamlining care pathways, and ensuring patients receive appropriate and timely care. Your role will be crucial in managing patient flow and optimizing healthcare delivery. Key Responsibilities: * Conduct initial patient assessments, including taking medical histories and performing physical examinations. * Streamline patient care pathways by determining the urgency and appropriate level of care needed. * Collaborate with multidisciplinary teams to ensure coordinated and effective patient care. * Maintain accurate and comprehensive patient records. * Provide patient education and support regarding their health and care plans. * Adhere to all relevant clinical guidelines, policies, and legislation.
