Support services coordinator

Support Services Coordinator 9 months Full Time Durham based role GBP12 - GBP14 per hour Exciting opportunity to work with one of my local social housing clients. The role To provide responsive, high quality, support to clients, in order to promote independent living, as appropriate; clients can be defined as individuals, families or children that might have a range of diverse needs. + To support clients, in a flexible and person centred way, utilising PSI interventions and delivering practical tasks, which address their needs and aspirations, through implementing and authorising support/ treatment plans. + To undertake, authorise and review individual client needs assessment which generates an Outcome Star which shows the distance travelled and outcomes achieved by the clients. + To undertake, authorise and review individual client risk assessments through effective communication and rapport building with the client. + To be accountable for adherence to internal targets and performance measures.
