Available Team Roles; Adults Safeguarding Team Adults Mental Health Team Adults Duty Assessment Team Adults Locality Team Adults Hospital Discharge Team Day To Day Responsibilities Building and maintaining strong relationships with Adults their carers, utilising a care centred approach. Accurately logging maintaining data within your caseload, to a standard to be presented in court. Making sure to be proactive forward thinking, when completing safeguarding assessments. Ensuring that practise procedures fall in line with modern legislation council policies. Liasing with other professionals such as homes, hospitals, police etc. Essential Requirements Social Work Degree with registration with Social Work England. Minimum of 3 Years Post Qualifying Experience, after completion of ASYE. Strong work ethic, with competency in handling a varied complex caseload. Care centred solution driven approach. High level report writing skills. Self-motivation self-discipline.
