Implement various Discharge to Assess and re-ablement pathways to promote independence outside of the hospital environment. Conduct assessments to identify immediate and long-term care needs, supporting individuals in achieving their care goals. Collaborate with a multi-agency team to ensure effective, safe, and timely discharges across different hospital sites. Conduct Mental Capacity assessments, participate in Best Interest Meetings, and attend MDT meetings as required.
