We have an excellent opportunity with the Wandsworth Borough Council #39;s Leaving Care Team. As a Specialist Mental Health Personal Advisor, you #39;ll be part of a dynamic team dedicated to helping young people build stable and successful lives. What You #39;ll Do: Experience: Utilise your proven expertise in managing challenging behaviors and engaging young people with mental health issues. Regulatory Compliance: Demonstrate sound knowledge of key legislation such as the Leaving Care Act 2000 and Social Work Act 2017 while working with young people both in outreach and in-office settings. Caseload Management: Be the guiding hand for 15 young people with moderate to acute mental health challenges, helping them navigate towards stability and independence. Pathway Plans: Craft detailed Pathway Plans for your caseload, supporting them every step of the way to achieve their goals. Emotional Wellbeing / Mental Health: Collaborate with our Therapeutic Service and transition social worker, fostering positive emotional well-being and mental health among young people. Multidisciplinary Team: Work closely with Adult rsquo;s Social Care Social Workers and Care Coordinators to provide holistic support and access to appropriate mental health services.
