Key Responsibilities: Assess and prioritize emails received by Social Care Direct, furnishing insightful notes and guidance to team members on case direction and subsequent actions. Methodically allocate emails to team members, ensuring swift and equitable distribution of workload. Provide invaluable support and counsel to team members regarding assigned emails, aiding in informed decision-making and effective case management. Identify pertinent processes and pathways such as reablement, telecare, OT, Care Act Assessment, safeguarding, and hospital discharge, and offer appropriate signposting. Diligently screen incoming emails to ascertain suitability for referral to Social Care Direct, maintaining meticulous attention to detail and adherence to established protocols. Requirements: Hold professional qualifications in Social Work and be registered with Social Work England. Demonstrate proficiency in social care practice, particularly in triaging and directing referrals. Possess strong organizational acumen, adept at managing email correspondence and effectively allocating workloads. Exhibit exceptional communication skills, capable of delivering clear guidance and support to team members. Display proficiency in relevant processes and pathways within adult social care, including reablement, telecare, OT, Care Act Assessment, safeguarding, and hospital discharge.
