Job Summary: We are seeking compassionate and dedicated Support Workers to provide care and assistance to individuals in need. As a Complex Needs Support Worker, you will play a vital role in improving the quality of life for our clients by providing essential support and companionship. This is a rewarding opportunity to make a positive impact on the lives of others. Our Client provides accommodation opportunities for adults who have experienced a variety of complexities and inequality. As a Support Worker will need to be skilled at building positive relationships, deliver appropriate emotional and practical support to individuals experiencing homelessness and with multiple and complex needs. Ensure that individuals with complex needs receive enhanced support to access treatment and wider services as part of their journey. Location: Hitchin Hours: 7am to 3pm (early) or 1pm to 9pm (late) for 5 days out of 7 totaling 40 hours per week. Rate: GBP11.44ph Contract: Temp to Perm (12 weeks) Responsibilities: * Providing support services related to housing for individuals who are currently experiencing rough sleeping or facing the risk of homelessness. * Assisting individuals through a structured support process and appropriate interventions to facilitate a successful transition into community housing. * Coordinating referrals for new service users through community partners, offering guidance and information on service delivery plans, suitability, and the referral procedure. * Collaborating with internal and external partners, agencies, and commissioners, including Rough Sleeping Outreach Teams, Supported Accommodation, Probation services, Drug and Alcohol Services, Local Authority Housing teams, and other community-based support providers. * Offering accommodation support to help individuals maintain their tenancy, whether by supporting them in their current home or during the transition into the service. * Managing a caseload of service users from initial assessment, developing personalized outcome plans, and regularly reviewing progress towards achievable move-on options. * Developing comprehensive risk assessments to address individual safeguarding concerns. * Working closely with the team and Service Manager to monitor the progress of service users on the caseload. * Engaging in partnership with relevant professionals, agencies, and providers to ensure appropriate support is accessible and delivered to service users. * Facilitating effective communication among all partners to achieve desired outcomes for individuals. * Enhancing the availability of private sector accommodation and increasing the likelihood of successful transitions into private sector tenancies. * Collaborating closely with partner agencies and support providers, ensuring clear communication and information sharing in alignment with Keystage Housing and legislative data protection and confidentiality standards. * Documenting all interactions with service users, including updating resettlement plans, contact details, and risk assessments as needed. * Supporting service users in attending community appointments, promoting social inclusion, and facilitating transitions into further accommodation. * Maintaining and updating records in accordance with internal and regulatory policies and procedures. * Establishing positive partnerships with the local authority, service providers, and agencies, directing service users to relevant resources to enhance participation, health, wellbeing, social inclusion, volunteering opportunities, and pathways to employment. * Actively participating as a team member to uphold high standards and pursue continuous improvement, attending regular meetings and briefings. * Occasionally working outside normal office hours as required to fulfill the Group #39;s commitment to equal opportunities and non-discriminatory practices. * Adhering to Section 7 of the Health and Safety at Work Act by taking reasonable care of personal health and safety as well as that of others who may be affected by actions or omissions. * Undertaking additional duties as reasonably appropriate for the level of the post.
